Clinics & Services

For All Long term Conditions please fill in the Questionnaires on https://systmonline.tpp-uk.com/ prior to review.

If you have a home blood pressure monitor please have the average of 1 weeks’ worth of readings ready for your review. BP monitors can be purchased on line or bought in a local chemist. If you are unable to use a monitor we will discuss this at your review.

If you have online access https://systmonline.tpp-uk.com/ please go on line and complete the Lifestyle questionnaire and also the questionnaire relevant to your condition. If you do not yet have online access please contact the surgery to set this up. This makes it easier for your reviews and ordering your medication.

Our team of practice nurses and healthcare support workers work alongside the GPs and hospital consultants to help monitor and manage our patients with long term conditions.

We routinely invite patients to come in for reviews on the month of their birthday which can involve:

    • Blood pressure checks
    • Height & weight monitoring
    • Blood tests

Asthma

We recommend that all patients with asthma have regular reviews to assess their treatment and to ensure they are on the appropriate medication.  We will invite you annually for this review (children under the age of 16 years every 6 months for a review). If you are well controlled we may do this review over the telephone.

Diabetes

We recommend all patients with diabetes have an Annual review with the practice nurse, 2 weeks prior to the review appointment we ask that you have a routine blood test (annually  blood test).

Heart Disease

We recommend all our patients with Coronary Heart Disease have an annual review with our practice nurses, prior to this review we would suggest a  blood test with our Health Care Support Workers about 2 weeks before.

Hypertension 

We recommend all patients with high blood pressure attend the practice for a BP check, this can be done by our Health Care Support Workers.

There are two types of high blood pressure:

  • Primary high blood pressure (also known as essential high blood pressure). There is no identifiable cause for this type of high blood pressure.
  • Secondary high blood pressure,where high blood pressure is caused by an underlying cause, such as kidney disease, or a particular type of medication that you are taking.

COPD

We recommend all patients with COPD attend the practice for an annual review with the practice nurse.

It is recommended that patients with any of the above have an annual flu jab – our clinics run from September, please look out for our notices as we will not send out invites for this vaccine.

NHS Health Checks

We offer a free NHS Health Check for adults in England between the ages of 40 and 74, at the check we will take your family medical history, check basic data such as your height, weight, blood pressure and pulse, offer any advice about smoking and weight loss if appropriate and do a simple blood test to check your cholesterol, which will help you by assessing your risk and giving you personalised advice on how to reduce it.

We will contact patients by letter when they are due to have the test and strongly recommend that all patients take advantage of this free check.

Travel Advice

If you are travelling abroad, you may need special immunisations. Please ask at reception for a travel form at least 6 weeks before you travel to enable the nurses’ time to review your medical records and order any vaccines. Your appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
