Doctors

Our team or Dr’s and Nurse Practitioners are now using the AskmyGP Service to triage all of our patients. This is a Fast, Personal and Secure way to assess your needs. Please use the link on this website to connect. For our patients who do not have access to the internet the patient navigators will assist with this process. Nursing Team

Our Nursing team undertake all the Chronic Disease Management reviews and you will receive an invitation for this on the month before your birthday. Please can you assist the Practice Nurses by filling in the questionnaires on https://systmonline.tpp-uk.com/if you have online access. Please  complete the Lifestyle questionnaire and also the questionnaire relevant to your condition. If you do not yet have online access please contact the surgery to set this up. This makes it easier for your reviews and ordering your medication.

If you have a home blood pressure monitor please have the average of 1 weeks’ worth of readings ready for your review. BP monitors can be purchased online or in a local chemist. If you are unable to use a monitor we will discuss this at your review.
