Our Nursing team undertake all the Chronic Disease Management reviews and you will receive an invitation for this on the month before your birthday. Please can you assist the Practice Nurses by filling in the questionnaires on https://systmonline.tpp-uk.com/if you have online access. Please complete the Lifestyle questionnaire and also the questionnaire relevant to your condition. If you do not yet have online access please contact the surgery to set this up. This makes it easier for your reviews and ordering your medication.
If you have a home blood pressure monitor please have the average of 1 weeks’ worth of readings ready for your review. BP monitors can be purchased online or in a local chemist. If you are unable to use a monitor we will discuss this at your review.
A lot of the reviews can be undertaken over the phone and the nurse will advise you what is needed.
In addition the nursing team run travel clinics, childhood immunisation programs, cytology clinics.
Our Health Care Assistants support the nurses and in addition carry out dressings, suture removal, ear irrigation, and phlebotomy.

What is a Patient Navigator?
A patient navigator helps to facilitate the patient’s journey, this journey can take many different pathways.
They provide help and direction to the appropriate services – this could be via our appointment system, patient specific paperwork (referrals, registrations etc), support for carers and much more. The team include:

The dispensary is open Monday to Friday 08:30 -18:30 for the collection, ordering and any queries regarding medication.
The dispensary telephone enquiry line is open 09:00 – 17:00, please note this is for medication queries only. Unfortunately, due to possible errors we cannot take requests for repeat medication over the telephone.
We cannot give repeat medicines indefinitely without reviewing your condition. The length of time between reviews will depend on your condition. Therefore, we may ask you to make an appointment with the doctor or nurse before we issue another prescription.
