We are dedicated to helping our patients live independently and supporting the friends, family and loved ones who help this happen every day.

Our Care Coordination team are on hand to help in the community to provide the support needed when you need it most.

Who is a carer?

Do you look after a relative or friend who due to ill health, physical or mental illness, disability, frailty or addiction cannot manage without your support?  Then you are a carer and may want to talk to someone about your caring role and any support needs you may have.

Anyone can become a carer.  Carers come from all walks of life, all cultures and can be any age. If you are supporting a loved one, neighbour, friend or relative on a regular basis, helping with the daily activity that person can no longer do on their own, then you are an ‘informal carer’.

If you hear your health professional talking about a ‘formal carer’ they are referring to a person who is employed by a company and visits individuals to support them as part of their employment.

A young carer is someone under the age of 18 who is caring for another person.
